Please send the output of

  find /mnt/loop

Probably the IFO files are named in a weird fashion. dvd::rip tries the
following globbing mask to find them:

/mnt/loop/{video_ts,VIDEO_TS}/{vts,VTS}*{ifo,IFO}
Doesn't seem to be the problem. See output of find command.

These are two different processes in dvd::rip. dvd::rip is always able to extract the subtitle images using subtitle2pgm from the subtitleripper package.

But subtitle2vobsub (from the same package) requires the original .IFO files of the DVD in place. dvd::rip tries to copy them when you read DVD
toc (by a simple mount and file copy).
*mount* and copy?
The latter fails for some reason
on your system, and thus subtitlevob2sub can't work because of the missing .IFO files.

I tried playing around with subtitle2vobsub. All I could ever get it to do was produce was:

[dave@bend ~]# subtitle2vobsub -vi /mnt/loop/VIDEO_TS/VTS_11_0.IFO
reading IFO file was successful
sub file movie_subtitle.sub removed
idx file movie_subtitle.idx removed
Using width:720 height:480 language:en vobsub index:0

[dave@bend ~]# cat movie_subtitle.idx
# VobSub index file, v7 (do not modify this line!)
#
# Generated by MPlayer 0.90pre6-2.95.3 (subtitleripper)
# See <URL:http://www.mplayerhq.hu/> for more information about MPlayer
# See <URL:http://vobsub.edensrising.com/> for more information about Vobsub
#
size: 720x576
palette: 008600, 54814e, ff7bff, 008500, 808080, 808080, 808080, 808080, 808080, 808080, 808080, 808080, fc7cff, 008600, ff79ff, 808080
# ON: displays only forced subtitles, OFF: shows everything
forced subs: OFF

# Language index in use
langidx: 0

id: en, index: 0

subtitle2vobsub would apparently hang at this point (no other output, no CPU utilization). movie_subtitle.sub was empty (zero length file) for all .IFO files. Also, VIDEO_TS.IFO gave:

[dave@bend ~]# subtitle2vobsub -iv /mnt/loop/VIDEO_TS/VIDEO_TS.IFO
Can't open IFO file
Opening ifo file failed. I tried to open v but got an error.
Using default or command line arguments for palette, width and hight instead

which shouldn't matter.

Let me know if there is any way to get more information out of subtitle2vobsub. I gave it varying amounts of time but always got the same result. Likewise, if this behavior is normal, let me know.

Cheers,
Dave
