Probably the IFO files are named in a weird fashion. dvd::rip tries the
following globbing mask to find them:
/mnt/loop/{video_ts,VIDEO_TS}/{vts,VTS}*{ifo,IFO}

These are two different processes in dvd::rip. dvd::rip is always able
to extract the subtitle images using subtitle2pgm from the
subtitleripper package.
But subtitle2vobsub (from the same package) requires the original .IFO
files of the DVD in place. dvd::rip tries to copy them when you read DVD
toc (by a simple mount and file copy).The latter fails for some reason
on your system, and thus subtitlevob2sub can't work because of the
missing .IFO files.

Another follow-up. I tried processing a different DVD and got the same
error message and same results regarding creating a vobsub file. The
.IFO files on the new DVD are in the correct directory and have file
names that match "/mnt/loop/{video_ts,VIDEO_TS}/{vts,VTS}*{ifo,IFO}".
